Health Plan
The Company recognizes the needs of employees for financial protection in the event of illness
or injuries that result in medical expense and loss of income. Providing adequate, cost-effective,
medical insurance protection is a concern of the company. The Company has selected a plan
designed to meet the employees' needs. The plan is financially subsidized by the company to
keep the employee's cost to a minimum.
The plan offered is somewhat complex. For this reason, it is suggested that the Summary Plan
Descriptions (SPD), or brochure that describes the plan, be referred to for specific information. A
staff member in the personnel department is available to answer specific insurance questions.
During new employee orientation, the cost, coverage, eligibility requirements, and conversion
privileges of the plan will be explained in detail. You will be provided with a copy of the
Summary Plan Description for the plan after you or your dependents become covered.
